Can only the enemy see it and can I do it without them there

Sorry for late response. You physically take on it's form, not just an illusion. If their fear is giant spiders you are a giant spider. If it's cabbage.. you're a cabbage. I'd reason you can save the most recent fear for transformation purposes.
