Hello there, Thanks for the report. If you are trying to unlock Dual Destiny, the witch does not appear just from reaching Stage 4 in a single Overthrow. You need to complete Overthrow in all three Pale Heart areas first, and then the Secret Keeper should begin appearing after the boss is defeated, usually with a short delay rather than instantly. So if you have only been repeating one area, you may just be missing a required step rather than doing anything wrong. That said, there have also been reports of the Secret Keeper/witch not spawning correctly for some players even when they believe they have done the required Overthrows, so if you have already completed all three areas and still never see the witch, that does line up with an issue others have reported. If you reply back with whether you completed Overthrow in all three regions before checking for the witch, that will help narrow down whether this is a missed requirement or the spawn issue.
