Haunted Altar has multiple soft lock
The boss bugs at half health. Or the tier 3 refuses to complete and the boss just never spawns.
Game randomly crashed while I was writing this post😭

Had this a few times, all the soft lock issues seem to be fairly consistently mitigated by slowing down. Instant melting of the shielded ogres or the main boss (first phase) blocks mechanics for progression. But if you don't insta-kill them, the mechanics work (more) consistently

For the moment, to workaround the softlock that can occur with the Headless One, which sounds like one of the softlocks you're encountering, we encourage you to wait 10 seconds until the Shriekers close in the first phase of the boss fight.
