Impossible to reach Ghost in GM Lake of Shadows
Ghost spawned on a ledge in the kill zone, completely unreachable, during a Grandmaster Nightfall.

If you went to boss and started the encounter I believe the ghost gets pulled but then you gotta get a res rather then deal dps. Unfortunate
