I believe it is "Rasputin 5" You find it At the top of the glass outside the Weapons Testing Lab in Rasputin's Bunker. (The Weapons Testing Lab is where you fight the giant Shank.

Yes it did. That's what it's done for millions of years. Find a civilization, give them a golden age, use them to defend it's self from the darkness, bail when they lose. Humans were losing, it was about to leave.

In the rasputin card it says it was about to or trying to leave and ras, determining that it staying was fundamental for human survival, hit it with a massive weapon to prevent it from leaving then went dark.
