Occasionally after dying, you will be stuck on the "Your light fades away screen." I think it is when you go into your inventory at a specific time after dying. If you are in a fireteam, your team can pull you out of it by starting the encounter, but if you are alone, the only thing you can do is travel to another planet to end the mission (or leave the raid/dungeon, etc) or close your game. This is on PC, and all my friends have this issue occasionally. I have noticed that if I get stuck on this screen during a mission on the moon, and I try to travel to the tower to unstuck myself, it brings me to a landing zone on the moon as opposed to the tower.
