Sometimes warlock melee charges just disappear
Just happened the other day in crucible. Arcane needle with lightning surge. I verified that I had one charge. I slid and hit the melee button, but nothing happened, and the charge was gone. Slid again and it looks like the icon changed color, like it's still there, but it's not.
I feel like something similar to this exists with glaives but can't really put my finger on it.

If you continue to experience this, then please consider using your platform's share feature to capture a videoclip of what happens. If you record a clip, you can upload it to a site like Imgur or Youtube and link us to it here. In addition to the clip, please let us know if you only seem to experience this in Crucible. We'll then investigate.
