Swift Whisper - Flawless Whisper Triumphs
See the linked video, finished The Whisper in [b]10:28[/b], Flawless, no skipping or traversal skips, and I've done this on Normal and on Expert and the Triumphs are still not unlocking or activating.

Done through the Legends area in the World Map, and also through Pinnacle Ops.

They're aware of it, but there isn't an ETA on when they'll be issuing a fix - the same issue also happens with Zero Hours time trial triumph. Realistically at this point it's bottom of the priority list and unlikely to be fixed anytime soon.
