Lightfall campaign triumphs not completing
I completed the Lightfall campaign on a single character. None of the individual Lightfall campaign mission triumphs (First Contact, Downfall, Breakneck, etc.) are marked complete.
I have replayed the missions via the Campaign replay menu on Normal and Legendary, allowed the mission to auto-orbit, and used the same character. Triumphs still do not update.
This appears to be an account-level triumph desync affecting the entire Lightfall campaign triumph set. Screenshots available if needed.
Requesting account review or confirmation whether this is a known unresolved issue.

Despite the 'any difficulty' bit, they only pop when playing those missions as part of the weekly rotation of replayable campaign missions, accessed from a node on the Neomuna map, on any of the difficulty options offered there. They will not pop when the missions are played from the campaign mission picker on Normal or Legendary. It was the same for Witch Queen's mission triumphs too, the year before.
