Portal grades and power drops bugged

I load up any matchmade activity and it tells me I'll get an A grade with loot at 514. But it never happens. Either I get a grade lower than A and 512 loot or I get an A but still loot at 512. Why is this happening?

Just did an Altars of sorrow which projected A-515. Got an A grade and all loot was 513.
