I have done encore multiple times, I've gotten all ratings, C, C+, B, B+, A and now A+ with ultimate difficulty being available and it still didn't drop, am I missing something? Some people are getting it by just completing the mission, or achieving A grade, but I just can't seem to get it to drop.
