I would have to say the MP helm my friends and I call it the downy helmet cuz it looks like a spartan with downs. You get the Mp helmet with the una boob chest and the Kat arm, and you've got yourself the stupidest looking combo on reach. Some others are really bad including the Emile helm but downy takes the cake.

HAZOP base is pretty fugly, but stick that rebreather attachment (CBRN/HUL) on and it transforms it entirely. I think the Military Police helmet is probably the worst though.
