I did know how to do it, that wasn't the issue. The guy who kept dieing was fireteam leader, so....

Speak up, call him a scrub and tell him to leave. If I wipe more than twice on totems and it isn't for a legit reason, I quit and find a new group. Some people are just down right scrubs.
