There is a scannable Coffin on the Devil's Spire, at the top during the Echo of Oryx fight, you can scan one of those coffin like items that make up the walls at the back. It's very hard to pin point and actually scan properly since the Echo is constantly attacking. It states the Ghost attempted to revive another, but he turned him down saying that he had been in enough wars for one life time. We were not the first choice...
