I just had a revelation about Halo.
"Killed by the guardians" happens when even the game doesn't know how you died.
Every time this happens, it means some glitched out, laggy guardian came over from Crucible and killed you with a Fist of Panic.

The guardians are their own thing in Halo, and are actually in Halo 5.
